I understand that it is my individual (or guardian’s) responsibility to inform the staff of Splash Surf Company Ltd of any medical condition or injury that may affect my safe participation within my chosen activity (‘s). Please note any medical conditions below. State none if not applicable.

Are able to swim 50m in light clothing

If you cannot swim 50 meters it may not prevent you from taking part in some activity but staff must be made aware

Photography and Videography Consent

In accordance with our child protection policy, we will not permit photographs, video or other images of young people to be taken without consent. If the child is under 16, consent must be obtained from a parent / carer. (If the child is over 16, it’s good practice to inform parents that photographs and/or videos of their child may be used if the child has given consent.) Splash Surf Company LTD will take all steps to ensure these images are used solely for the purposes for which they are intended. If you become aware that these images are being used inappropriately, please inform us immediately.” Do you consent to photography and video of your child for analysis, development and marketing purposes?

Participation in adventure sports entails some risk of injury. Splash Surf Centre Ltd staff are trained and appropriately qualified to run all sessions and at all times proceed in a manner to limit the risk of injury. However I accept that accidents and injury may occur. Any person’s behavior, which in the opinion of the staff of Splash Surf Centre Ltd may cause injury or jeopardies the safety of other members of the group or staff of Splash Surf Centre Ltd may be excluded from further participation at the discretion of the supervising instructor. Splash Surf Centre Ltd cannot be held responsible for loss of personal belongings and equipment. Splash Surf Centre Ltd can take and use images taken on activities for marketing and social media use, unless other wise requested not to Splash Surf Centre Ltd safety policy and insurance certificate is available for inspection upon request. Splash Surf Centre Ltd safety management systems and insurance certificate is available for inspection upon request.
